Job summary

Sheffield City Council’s Living and Ageing Well service is recruiting an Adult Social Care Practitioner for the Communities North Team. The post involves managing a varied caseload, undertaking Care Act assessments, planning, reviews and safeguarding work to support adults aged 65 and over in Sheffield.

You will work with families and carers, using a person-centred, strengths-based approach, undertake home visits, and collaborate with health professionals and partners to promote independence and

Qualifications

• NVQ Level 3 or equivalent qualification.
• Evidence of ongoing professional development.
• Understanding of social care legislation, safeguarding and person-centred practice.

Responsibilities

• Undertake Care Act assessments and identify eligible needs.
• Develop and review care and support plans with people and carers.
• Conduct reviews to ensure support meets identified needs.
• Identify, assess and respond to risk and safeguarding concerns.
• Apply Mental Capacity Act 2005 principles and contribute to best-interest decisions.
• Promote independence by linking to networks and community resources.
• Collaborate with health professionals, voluntary organisations and partners.
• Provide information, advice and appropriate signposting.
• Maintain clear, accurate records and document rationale for decisions.
• Undertake home visits across Sheffield.
