Job summary

Wokingham Borough Council is seeking a Social Care Practitioner for the West Team in Adult Social Care. This permanent, part-time position involves supporting adults to live safely and well in their communities, with a focus on strengths-based assessment and person-centered planning.

You will work under supervision from a social worker or occupational therapist and contribute to a high-performing service. The role offers a development pathway to Grade 6 and access to a wide range of staff

Qualifications

• Strong value base; respectful and trustworthy.
• Good IT skills and ability to use adult social care recording systems.
• Able to engage with people with different communication needs.
• Experience in health or social care or in a people-facing role.

Responsibilities

• Work with people to identify what is important to them.
• Carry out strengths-based assessments and reviews of daily lives and support arrangements.
• Gather information from individuals, carers, professionals and advocates.
• Write reports and keep accurate records.
• Build positive relationships with residents and support various communication needs.
• Work closely with colleagues as part of a team.
