Clinical Pharmacist Job Plan
Position: Clinical Pharmacist (Primary Care - Remote Medication Reviews)
Total Hours per Week: 12 hours
Core Responsibilities:
- Remote Medication Reviews:
- Conduct remote medication reviews with patients primarily via phone.
- Review and assess patients' current medication regimens, ensuring they align with best practice guidelines.
- Identify potential medication issues (e.g., drug interactions, inappropriate prescribing, non-adherence) and recommend improvements.
- Discuss the patient's understanding of their medication and provide education on proper use, side effects, and lifestyle considerations.
- Offer tailored advice on over-the-counter products, alternative treatments, and therapy adjustments.
- Document all recommendations and patient feedback in EMISweb.
- Appointments:
- Prioritize appointments based on patient need, medication complexity, and potential health outcomes, managed by the supervising team.
- Ensure each review includes medication history, adherence assessment, adverse effects identification, and suggestions for improvements within your clinical competency.
- Administrative Duties:
- Accurate clinical documentation in EMISweb.
- Follow-up communication via text or reception as needed.
- Communicate with GPs and the on-call doctor for concerns.
- Maintain logs for quality assurance and reporting.
- Stay updated with clinical guidelines and practices.
- Patient Follow-Up & Feedback:
- Address patient questions about medication management.
- Advise patients needing non-medication appointments to complete a triage form.
