Job summary

Early Years Alliance is seeking a Learning and Disabilities Practitioner to support children aged 0-5 with disabilities or complex health needs and their families across Lincolnshire. The role offers a rewarding chance to join a dedicated team providing the Children with Disabilities Support Service in Stamford, Spalding, Holbeach and Boston.

You will plan and facilitate sessions, monitor progress against EHCP goals, and work within an integrated team with social care and educational partners,

Qualifications

• NVQ Level 3 in Early Years/Childcare or equivalent, with relevant experience with disabled/complex needs.
• Able to communicate effectively with professionals and families.
• Experience working as part of an integrated team with social care and education.

Responsibilities

• Facilitate high-quality Early Support and Learning provision for birth to 5 years in Best Start centres while providing parents and caregivers a suitable break from their caring responsibilities.
• Plan and facilitate groups and sessions with activities promoting self-confidence and independence.
• Assess and track children's progress against EHCP outcomes using tools like Tapestry.
• Support parents to access Local Offer and promote peer group participation and engagement.
