Job summary

Early Years Alliance is seeking a Learning and Disabilities Practitioner to deliver high-quality early support and learning sessions for children aged 0-5 with disabilities or complex health needs. You will work in a supportive, integrated team across Lincolnshire and contribute to planning, delivering and evaluating activities aimed at building confidence and independence for children and families.

• Proven effective communication skills in person and in writing.
• Experience working in an integrated team with social care, early help teams and providers.

Responsibilities

• Facilitate high-quality Early Support and Learning provision for children birth to 5 with disabilities or complex needs in Best Start centres.
• Plan and run sessions that promote self-confidence and independence in a safe, welcoming environment.
• Track children's progress and outcomes using EHCP-related metrics and tools such as Tapestry and Outcome Wheels.
• Support families to access Local Offer and promote peer group participation.
