Job summary

Horton Housing is seeking an Early Intervention and Prevention Coach for our Halifax-based scheme on a permanent full-time contract. The role involves supporting young people (16-25) with brief interventions, referrals, and goal setting, with strong collaboration across agencies.

You will process referrals, provide strength-based assessments, and assist with housing registrations, grants, and healthcare access. A driving licence and DBS are required, and a 6‑week leave entitlement applies.

Qualifications

  • Understanding of challenges faced by young people (homelessness, ill-health, offending, unemployment).
  • Experience with strength-based, person-led approaches to working with people.
  • Ability to build effective working relationships and develop partnerships.
  • Experience in developing action plans and providing coaching/guidance.
  • Understanding of trauma-informed practice.
  • Effective communication and IT skills.

Responsibilities

  • Process referrals and carry out strength-based assessments.
  • Help register with housing providers and apply for CLS grants.
  • Support access to healthcare, benefits, education and training.
  • Maintain regular contact to track housing applications and current circumstances.
  • Collaborate with agencies to ensure appropriate services are accessed.
  • Assist young people to identify goals and begin achieving them.
  • Support from an apprentice and volunteers in the role.

Job Introduction

Are you passionate about supporting young people to identify their individual goals and working alongside them to achieve these goals ? Then this role is for you!

Horton Housing are currently recruiting for an Early Intervention and Prevention Coach to join our established scheme based in Halifax on a permanent full time contract. Working hours are between Monday and Friday (35 hours per week) and the annual salary for this role is £26,406.00. You will be entitled to 6 weeks paid annual leave (plus bank holidays) and full access to our Employee Assistance Programme.

The successful candidate will be the first point of contact for young people (16-25) accessing the service and be responsible for providing low level support and brief interventions. This will include processing referrals and carrying out strength-based assessments. It also includes help registering with housing providers, applying for CLS grants and support to access healthcare, benefits, education and training. You will be responsible for keeping in touch on a regular basis to check the status of housing applications, check people's current circumstances and crisis intervention. You will work closely with several agencies to ensure that young people are involved with the appropriate services and receiving the necessary support. You will help young people identify their individual goals at the assessment stage and work alongside them to start achieving these. You will be supported in your role by an apprentice and volunteers.

Essential skills and experience for the role:
  • Understanding of and/or experience of the challenges faced by young people going through difficult times e.g. homelessness, ill-health, offending, unemployment etc. This could be through paid employment volunteering or lived experience
  • Understanding of and/or experience of strength-based, person-led approaches to working alongside people
  • Ability to build effective working relationships
  • Ability to develop and maintain strong partnership working; liaising and working closely with other agencies/organisations to provide support
  • Experience of developing action plans and providing appropriate coaching, advice and guidance would be desirable
  • Understanding of trauma informed practice would be desirable
  • Effective communication and IT skills
