Job description

Job Overview

The post holder will support trained nurses and other healthcare professionals in delivering high-quality, patient-focused care.

Improve the fundamental aspects of health and social care for individuals who are acutely or chronically ill, frail, or disabled.

All HCSW posts require completion of Level 2 NVQ within 30 months of appointment. It is mandatory for individuals to have completed the NVQ to the required level before a permanent support worker position is offered by the Health Board.

Main duties of the job

  1. Ensure the 12 fundamentals of care are applied, including communication, respecting people, safety, promoting independence, relationships, sleep, rest, activity, comfort, pain alleviation, personal hygiene, eating, drinking, toilet needs, oral health, and pressure sore prevention.
  2. Assist and support patients in maintaining quality of life and performing tasks they would normally do themselves or that a close relative would do.
  3. Undertake direct patient care tasks under supervision of a Registered Nurse.
  4. Assist nurses or team members in carrying out nursing procedures requiring two people.
  5. Maintain the patient care environment and undertake duties directly involved with patient care.
  6. Report any changes in patient condition or behaviour to the Registered Nurse or Team Leader.
  7. Display sensitivity to individual patient needs while preserving dignity.
  8. Respect and maintain patient and colleague confidentiality.
  9. Participate in and cooperate with the nursing team and multidisciplinary team to enhance team effectiveness.

The ability to speak Welsh is desirable; applications from Welsh and/or English speakers are equally welcome.
