Job summary

University Hospitals of Morecambe Bay NHS Foundation Trust is seeking a Band 5 Community Rapid Response Nurse in Furness. This temporary post supports maternity leave/secondment and focuses on delivering a 2-hour crisis response to acutely unwell patients in the local community.

You will join a multidisciplinary team at Furness Rapid Response, working flexibly to prevent unnecessary hospital admissions and to support the newly established Virtual Wards.

Qualifications

  • The role requires excellent time management and the ability to work under pressure.
  • Experience in discharge planning and clinical assessment is essential.
  • Knowledge of current national and local NHS activity in acute and primary care.
  • Registered Nurse with Band 5 (adult branch) registration is required.
  • Experience in primary health care teams and clinical audit is advantageous.

Responsibilities

  • Deliver urgent community care as part of Furness Rapid Response team.
  • Contribute to the 2-hour crisis response standard to prevent hospital admissions.
  • Work within a multidisciplinary team and use clinical judgment to plan care.

Furness Rapid Response are looking for a Band 5 Nurse to join our team in the community.

This is a temporary post to cover Maternity Leave / secondment opportunity.

We are seeking a hardworking, dynamic, proactive, individual to join our well established multi disciplinary team.

You will support the delivery of the urgent community response standard which aims to provide a 2-hour crisis response to patients who are acutely unwell within our local community to avoid unnecessary hospital admissions. As well as working within the newly established Virtual Wards.

This is can be a challenging role , every day is different but exciting and rewarding.

Main duties of the job

The role requires fantastic communication skills, creative thinking and working proactively and collaboratively with others to ensure a high standard of care is delivered.

The role requires undertaking holistic assessments sometimes in complex and challenging situations to develop and deliver robust programmes of care for patients and carers. This is an exciting time to work in the community. The community is advancing and developing the urgent care response to prevent un-necessary hospital admissions.

You must be able to use your own initiative, work under pressure, keep to deadlines and have excellent nursing, organisational, interpersonal and communication skills. You should be flexible and able to work as part of a team.

We will provide a programme of induction and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.

With patient centred care as your main focus, you will deliver our core values, eg respect, empathy, consideration, compassion and dignity.

You will be joining an enthusiastic and motivated team who are friendly and approachable. The team works flexibly together to maintain support for each other and to meet our service delivery.

Working for our organisation

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
