Job summary

Gateshead Health NHS Foundation Trust is seeking a Band 6 Nurse Practitioner to join the Rapid Response Nursing Team on a part-time basis (30 hours per week). This role involves delivering high-quality, patient-centred care in the community, with shifts including day, night, twilight, weekends and Bank Holidays.

You will work autonomously within the MDT to assess and intervene for adults at home, contribute to 24/7 rapid response delivery, and support admission avoidance while maintaining safety

Qualifications

• Diploma/Degree in Adult Nursing required.
• Accredited clinical skills or equivalent expert knowledge.
• Teaching qualification or equivalent advantageous.
• Experience with PGDs and working with community services.
• Full driver's licence required.

Responsibilities

• Provide autonomous clinical interventions in the community.
• Deliver urgent and planned nursing care at patients' homes.
• Support admission avoidance and patient pathways.
• Lead and supervise MDT colleagues as directed.
• Ensure governance and patient safety in service delivery.
