Job summary

Gateshead Health NHS Foundation Trust is seeking a Band 6 Nurse Practitioner to join the Rapid Response Nursing Team in a part-time community role. Qualifications

• Diploma/Degree in Adult Nursing is required.
• Accredited Clinical Skills/or equivalent expert knowledge.
• Teaching qualification or equivalent.
• Evidence of working with PGD's.
• Full Drivers License.

Responsibilities

• Assess, plan, implement, and evaluate nursing care to meet the individual needs of patients, families, and carers within community settings.
• Deliver high-quality, evidence-based nursing care to adults within their own homes and other community environments.
• Undertake and manage planned and unplanned clinical interventions, working autonomously and within agreed professional competencies.
• Provide timely assessment and treatment of patients with complex health needs, including frailty, long‑term conditions, palliative care, and end‑of‑life care requirements.
• Ensure patients receive the right care, at the right time, in the right place, delivered by appropriately skilled professionals.
