Job summary

Via Harrow is seeking a Dual Diagnosis Assertive Outreach Recovery Practitioner to join our Harrow service. The role focuses on delivering high-quality care to individuals accessing treatment across the borough, with emphasis on opioid use and poly-substance use in complex cases.

You will work within a multidisciplinary team, coordinating with GPs, CMHTs, hospitals and safeguarding teams to promote safe treatment and wellbeing.

Responsibilities

• Deliver an effective and safe service covering duty, referrals and assessments.
• Manage a caseload including opiate substitute prescribing and relapse management.
• Develop and deliver group sessions with a creative team.
• Liaise with GP, CMHT, Hospitals and Safeguarding to promote safe treatment and wellbeing.
• Engage in outreach to increase referrals from mental health services within Harrow.
