Job description

Overview

Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives.

Our core values are 'Be open, be compassionate and be bold' and our teams apply these daily offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options.

RISE (a consortium, led by Change Grow Live, and includes Central and North West London NHS Mental Health Trust, and service user-led organisation Build on Belief) is the integrated end-to-end treatment and recovery service which offers a wide range of pharmacological and psychosocial interventions to Ealing residents 18 years and over.

The Dual Diagnosis Team (Co-existing Mental Health and Substance Use) for RISE supports service users dealing with complex co-existing mental health and substance use conditions. A dedicated pathway of care will enable collaborative delivery of support by multi-agency teams in response to their needs in order to create & deliver combined treatment plans which are appropriate & effective in addressing their substance misuse issues.

As RISE continues to increase the provision of support, access, and assessment for individuals who due to their complex mental health condition(s) have historically been unable to engage and/or be retained in substance misuse treatment, a new post has arisen in this Team for a specialist Acute psychiatric ward Dual Diagnosis Engagement Worker role.

We're looking for a committed professional to support individuals with complex co-existing mental health and substance use needs. Many of our clients face additional challenges such as neurodevelopmental conditions, learning disabilities, unstable housing, poor health, safeguarding concerns, and limited support networks.

You’ll manage a dedicated caseload and work flexibly across the community and offering assertive in-reach support in Hammersmith & Fulham Mental Health Unit, Charing Cross Hospital.

This is a mobile, hands‑on role suited to someone resilient, empathetic, and motivated to drive change where it matters.

Where:

Ealing Borough and Hammersmith & Fulham Mental Health Unit Claybrook Road London W6 8NF

Hours:

Part Time, 22.5 hours per week

Contract:

Fixed Term until 31st March 2027

Full‑time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities
About the role:

In this role, you will have the opportunity to support individuals with co‑existing mental health and substance use challenges, who may also experience additional complexities such as neurodevelopmental conditions, learning disabilities, physical health issues, lack of community or family support, or unsuitable living conditions. You will work to overcome barriers to engagement, helping individuals connect with meaningful support, and addressing safeguarding and exploitation concerns.

Working as an integral part of a compassionate, multi‑disciplinary team that includes Recovery Practitioners, Doctors, Nurses, Psychiatrists, and Psychologists, you will support each individual's journey to recovery from their initial contact with RISE through to ongoing treatment.

Your role will include:
  • Conducting comprehensive assessments and screenings
  • Offering tailored pharmacological and psychosocial interventions, including cognitive‑behavioural approaches, motivational interviewing, and recovery planning
  • Providing support that reduces drug and alcohol‑related harm, and fosters greater involvement from family, carers, and the wider community
  • Making onward referrals that connect individuals with the right services for their continued recovery

You will work flexibly across both community and inpatient settings, using an assertive outreach approach to meet people where they are. This includes visiting individuals in locations such as Acute Psychiatric Wards, Substance Misuse Hubs, Community Mental Health Services, GP Surgeries, and Supported Accommodation.

The post holder will need to work flexibly across the local community and inpatient settings with hours to suit service need.

The team is peripatetic rather than hub based with the post holder expected to conduct assessments & reviews using assertive in‑reach in a variety of venues - including Acute Psychiatric Wards, Substance Misuse Hubs, Community Mental Health Services, GP Surgeries, and Supported Accommodation.

About You:
  • You will bring empathy, patience, and a strong commitment to person‑centred support and treatment.
  • You are flexible, with the ability to work across different community settings, and adapt your approach to meet the unique needs of each individual.
  • You have experience in supporting people with complex mental health and substance use challenges and are skilled at building trust and encouraging positive change.
  • Your approach is inclusive and trauma‑informed, ensuring that the people you work with feel heard, valued, and empowered.
