Job summary

Homecare Gurus is seeking a Care Coordinator to support high-quality domiciliary care services in the High Wycombe area. You will work with carers, service users and families to ensure person-centered plans and timely delivery of care, promoting independence and wellbeing.

The role involves organising rotas, liaising with healthcare professionals, attending reviews, and maintaining compliant records under GDPR and safeguarding requirements.

Qualifications

• Level 2 or 3 qualification in Health and Social Care (or working towards).
• Experience as senior carer or care supervisor in domiciliary care.
• Familiarity with care planning software and digital record-keeping.
• Knowledge of safeguarding, Care Act 2014, and care regulations.
• Full UK driving licence.

Responsibilities

• Coordinate care plans and reviews to reflect Service Users’ goals.
• Support rota creation and match carers to tasks and locations.
• Respond to changes in needs and cover shifts as required.
• Record observations and maintain care documentation per regulations.
