Job summary

Horizon Homecare seeks a proactive Hybrid Care Coordinator to join our branch team in delivering high-quality domiciliary care. You will coordinate schedules, match workers to clients, and support branch growth while maintaining compliance and documentation.

Key duties include rota production, on-call support, and building strong relationships with clients, families, and clinical partners. A driver with DBS is essential; join us to help improve lives daily.

Qualifications

• Right to Work in the UK
• Fluent English with excellent verbal and written communication skills
• Full UK driving licence and access to your own vehicle
• Flexibility to participate in the on-call rota, including evenings and weekends
• Enhanced DBS check (or willingness to obtain one)
• NVQ Level 3 in Health & Social Care (or equivalent)
• Previous experience in a Care Coordinator role

Responsibilities

• Coordinating care schedules and matching Care & Support Workers to clients based on skills, location and needs
• Producing accurate rotas and ensuring full coverage while responding to changes and emergencies
• Building positive relationships with clients, families, Support Workers and healthcare professionals
• Supporting the Registered Manager with branch growth by developing new care packages and maximizing capacity
• Recruitment, onboarding and induction of new Care & Support Workers
• Monitoring service quality and ensuring compliance with CQC standards and company policies
• Maintaining accurate records and up-to-date care documentation
• Participating in the on-call rota when required

We're looking for an experienced Care Coordinator to become the heart of our branch operations. You'll play a pivotal role in ensuring our care services run smoothly by coordinating staff, managing rotas, supporting our care teams, and delivering exceptional service to our clients.

What you'll be doing

• Coordinating care schedules and matching the right Care & Support Workers to each client based on skills, location and individual needs.
• Producing accurate rotas and ensuring all care visits are fully covered while responding quickly to changes and emergencies.
• Building positive relationships with clients, families, Support Workers and healthcare professionals.
• Supporting the Registered Manager with branch growth by developing new care packages and maximising existing capacity.
• Assisting with recruitment, onboarding and induction of new Care & Support Workers.
• Monitoring service quality, resolving concerns professionally and ensuring compliance with CQC standards and company policies.
• Maintaining accurate records and ensuring all care documentation is up to date.
• Participating in the on-call rota where required.

• Experience coordinating domiciliary care services.

• Confidence using care management systems and Microsoft Office.
• The ability to prioritise workloads and adapt quickly in a busy environment.
• A proactive, solutions-focused approach.
• A genuine passion for delivering person-centred care.
