Horizon Homecare, part of Natali Healthcare Solutions, is seeking a Care Coordinator for our Poole branch. This full-time role focuses on coordinating staff, managing rotas, and delivering high-quality in-home care with a £29,000 annual salary.

You will build strong relationships with clients and teams, support recruitment and onboarding, ensure CQC standards, manage records, and take on on-call duties as required. This is a busy, client‑facing position that makes a real difference.

Qualifications

• Right to work in the UK.
• Fluent English with excellent verbal and written communication skills.
• Full UK driving licence and access to your own vehicle.
• Flexibility to participate in the on-call rota, including evenings and weekends.
• Enhanced DBS check (or willingness to obtain one).

Responsibilities

• Coordinate care schedules and match the right Care & Support Workers to each client based on skills, location and individual needs.
• Produce accurate rotas and ensure all care visits are fully covered while responding quickly to changes and emergencies.
• Build positive relationships with clients, families, Support Workers and healthcare professionals.
• Support the Registered Manager with branch growth by developing new care packages and maximising existing capacity.
• Assist with recruitment, onboarding and induction of new Care & Support Workers.
• Monitor service quality, resolve concerns professionally and ensure compliance with CQC standards and company policies.
• Maintain accurate records and ensure all care documentation is up to date.
• Participate in the on-call rota and provide occasional hands-on support where required.

What you'll be doing
