Job summary

Oxford Health NHS Trust is recruiting for a District Nursing Lead to drive clinical leadership for housebound patient care across home, clinics and care homes. The post focuses on implementing the Ageing Well anticipatory care model, guiding teams through complex needs, and reducing hospital admissions through evidence-based pathways.

The role requires DNSPQ and non-medical prescriber eligibility, with strong collaboration across GPs and multi-disciplinary teams to improve outcomes and service

Qualifications

• Advanced history taking or comprehensive assessment of the person with frailty module
• District Nursing Specialist Practitioner Qualification (DNSPQ)
• Non-medical prescriber (V300) or willingness to undertake the module

Responsibilities

• Provide clinical leadership and champion best practice across District Nursing teams, aligning patients to the correct care pathways
• Ensure planned care meets high quality standards and evidence-based pathways
• Guide and support District Nursing teams in managing complex and advanced patient needs
• Lead the local implementation of the national Ageing Well anticipatory care model
• Champion preventative and personalised care
• Build integrated working with GPs, multi-professional teams and locality leads to reduce hospital admissions
• Carry out advanced clinical assessment and prescribing
• Develop care pathways
• Supervise and support staff to deliver safe, effective care
• Be a role model for professional standards and foster a culture of learning and quality across the service
