Salford Royal NHS Foundation Trust is seeking a District Nurse/Charge Nurse to join the Community Nursing Team in Rochdale. You will deliver high quality, patient-centred care in patients' homes and lead a diverse caseload with autonomy and clinical leadership.

The role requires strong assessment, care planning, and coordination with multidisciplinary teams to support independence and wellbeing; you will mentor colleagues and drive service improvements within a 7 days a week operation.

Formación

• Registered nurse with district nurse/charge nurse experience.
• Proven ability to lead in community care settings.
• Strong assessment and care planning skills.

Responsabilidades

• Lead district nursing visits in the community.
• Assess, plan, and coordinate patient care with MDT.
• Mentor and supervise junior staff and students.
• Ensure safe, effective and compassionate care for long-term conditions.
