Leeds Community Healthcare NHS Trust is seeking an enthusiastic Band 7 District Nurse to lead our Neighbourhood Teams in community settings. You will hold the Community Specialist Practitioner District Nurse Course with V300 prescribing and work to deliver patient-centred care in patients' homes.
The role requires clinical leadership and collaboration across multidisciplinary teams to prevent hospital admissions.
Competenze
- Must have completed Community Specialist Practitioner District Nurse Course with V300 prescribing.
- Current UK professional registration is required to practice.

- Manage a clinical caseload across neighbourhood teams.
- Provide clinical leadership and oversee a cluster of Staff Nurses and Clinical Assistants.
- Deliver high-quality, compassionate, proactive and safe community nursing care.
