Job summary

Qualifications

• Teaching qualification and district nursing prescriber status (V300 NMP).
• Proven ability to manage complex caseloads and lead teams.

Responsibilities

• Manage a caseload of registered patients within the district nursing team.
• Delegate workload to team members and liaise with other health professionals.
• Conduct comprehensive assessments and develop, deliver, and evaluate care plans.
• Perform chronic disease reviews for housebound patients and support service delivery.
• Lead on audits, documentation, and adherence to NMC standards.
• Support training and supervision of students and junior staff.
• Provide weekend rota coverage to ensure continuity of care.
