The Methodist Church in Britain offers a 30-hour part-time community nursing post based in Buckinghamshire, delivering care to patients in their homes. You will manage a caseload of individuals with long-term conditions, with a focus on preventing hospital admission and coordinating district nurse services.

The role requires registration with NMC, driving licence and access to a car, plus willingness to undertake District Nurse training and CPD to maintain clinical excellence.

Qualifications

• Nursing degree or diploma is required.
• Post-graduate clinical training in elderly care or long-term conditions is preferred.
• Evidence of recent continuous professional development (CPD).

Responsabilités

• Assess the physical and psycho-social needs of a defined caseload and start treatment and care.
• Manage a caseload of patients with long-term conditions who would benefit from case management.
• Deliver nursing interventions in patients' home environments.
• Provide IV therapy, catheter care, wound and leg ulcer care, palliative and end-of-life care, and medicines management.
• Undertake referral management to set up case management programmes.
