Procare Community Services is seeking a registered nurse to join its adult community nursing team. You will visit adults at home, assess and treat a caseload with tasks including wound care, catheterisation and end of life care, working under a District Nurse.

You'll work seven days a week, 8am to 8pm, with a planned review for a Band 6 senior post once competencies are met. Visa sponsorship is not offered.

Formación

• Registered nurse with NMC registration and current practising status.
• Experience in community/outpatient care and home visits preferred.
• Ability to work autonomously and manage a caseload.

Responsabilidades

• Visit adult patients in their own homes who have a nursing need, including long-term, complex and end of life care.
• Assess, implement and evaluate person-centred care for a caseload of patients.
• Carry out venepuncture.
• Provide wound care.
• Carry out catheterisation.
• Manage syringe drivers.
• Provide end of life care and symptom control.
• Prioritise visits according to patient clinical need.
• Organise own workload.
• Record care on the EMIS electronic patient record.
• Work as part of a local team with the acute hospital, social care, mental health teams, GPs and practice nurses.
• Work under the direction of a District Nurse.

Checks & eligibility

• Full UK driving licence and access to a vehicle for business use, where required for the post
• Independent right to work in the UK, as the employer does not sponsor visas
