Job summary

Midlands Partnership NHS Foundation Trust is recruiting a Band 5 Community Nurse for the Discharge to Access and Home First Service in north Staffordshire. The role focuses on safe discharge, community-based nursing, and coordinated care to prevent hospital admissions.

You will work within a multidisciplinary team to assess and plan care, support patients at home, and maintain high standards of clinical care across various community settings.

Qualifications

• Proven professional and clinical knowledge of community nursing.
• Competence as appropriate to the role.
• Demonstrate MPFT values in practice.
• Ability to deliver care in home, residential, or clinic settings.
• Supervisory responsibilities may be required.

Responsibilities

• Deliver care to patients in their own homes, residential homes, or clinics where appropriate.
• Be responsible for a defined population across community and hospital settings.
• Assess care/support needs and develop, implement and evaluate care plans.
• Work within a multidisciplinary team to ensure high quality care.
• May have supervisory responsibilities such as work allocation.
