Lincolnshire Community Health Services NHS Trust is seeking a Promoting Independence Therapy Assistant (Band 3) to support patients at home after discharge from hospital. The role emphasises rehabilitation and reablement to promote independence, assess needs, and help people return to their own homes.

You will join a team delivering Discharge to Assess principles across Lincolnshire, with shifts planned 0800–2000 seven days a week and the possibility of part-time work.

Competenze

• 5 GCSEs at grade 4 or above or equivalent.
• A Level 3 Vocational Healthcare Qualification or equivalent.
• Moving and handling training/experience.

Mansioni

• Provide rehabilitation and reablement to patients at home immediately after discharge.
• Promote independence and contribute to assessments to determine future needs.
• Work with the multi-disciplinary team to plan and progress care in the home environment.
