Job summary

Imperial College Healthcare NHS Trust is seeking an experienced, motivated nurse to join our complex discharge team. You will be predominantly based at Charing Cross Hospital but will work across sites as required, bringing strong communication and interpersonal skills, initiative and openness to feedback.

The post holder will manage a caseload of complex patients, participate in staff development, and provide weekend and bank holiday cover as part of contracted hours.

Qualifications

• Registered Nurse with current UK registration.
• Evidence of teaching or equivalent experience in hospital/community.
• Active engagement in professional development and national policies.

Responsibilities

• Deliver high standards of patient care and pathway management to reduce delayed transfers of care.
• Undertake/coordinate assessment of patients' needs.
• Act as SMH Discharge Hub screener on a rotational basis.
• Deputise for the Discharge Team Manager as required.
• Identify educational needs and deliver staff development opportunities.
• Line manage and supervise allocated junior staff.
• Work within the Discharge Lounge to ensure safe transfers home.
• Participate in weekend and bank holiday rota.
