Job summary

Imperial College Healthcare NHS Trust is seeking an experienced Integrated Discharge Hub Lead to oversee the discharge process across inpatient wards within Charing Cross. You will join the Integrated Discharge Leadership team to drive timely patient flow, manage the complex caseload and guide the discharge team through education, audits and improvement projects.

Qualifications

• Experience leading hospital discharge teams.
• Ability to plan patient flow and manage complex caseloads.
• Knowledge of discharge documentation and audits.
• Strong communication with multidisciplinary teams.

Responsibilities

• Lead and manage the complex discharge team on a day-to-day basis within Integrated Discharge Leadership.
• Deliver and oversee the complex discharge service, ensuring timely hospital flow.
• Plan efficiently for patient management and time use across wards.
• Support staff through supervision, training and professional development.
• Review and audit discharge documentation for quality and consistency.
• Provide education to clinical and nursing staff on discharge remit.
• Cover out-of-hours shifts as part of local rota.
