Job summary

Imperial College Healthcare NHS Trust invites applications for an experienced nurse to join the complex discharge team. Based mainly at Charing Cross Hospital, you will collaborate across sites to coordinate safe discharges and support families navigating the pathway.

You will promote education, workforce development, and participate in weekend/bank holiday cover while working within a multidisciplinary team to improve patient flow and outcomes.

Qualifications

• Registered Nurse with current UK registration.
• Teaching qualification or equivalent experience in hospital/community.
• Experience in acute settings and complex discharge processes.

Responsibilities

• Deliver high-standard patient discharge management and pathway coordination.
• Assess patients’ needs and coordinate services for safe, timely discharge.
• Act as SMH Discharge Hub screener on a rotational basis.
• Deputise for Discharge Team Manager at relevant meetings.
• Support staff development through education and supervision.
