Job summary

Imperial College Healthcare NHS Trust is seeking an experienced discharge leader to manage the complex discharge service within the Charing Cross Integrated Discharge Team. The role focuses on clinical leadership, coordinating complex discharges across inpatient wards, and ensuring timely patient flow.

You will work with the discharge team lead and contribute to trust-wide improvements, with scope for Quality Improvement projects and cross-site collaboration.

Qualifications

• NMC registration and mentorship qualification are essential.
• Experience leading discharge planning in a hospital setting.
• Ability to coordinate complex discharges with multiple teams.

Responsibilities

• Lead and manage the complex discharge service day-to-day.
• Provide clinical leadership and guidance to the discharge team.
• Plan, organise and optimise patient flow and discharge processes.
