Hertfordshire County Council is recruiting for a Team Manager in the Integrated Discharge Service, based in Apsley. The role leads a fast-paced, multi-disciplinary team delivering coordinated care for people post-hospital discharge, with a focus on Home First and independence.

You will ensure safeguarding, oversee performance, and support service development across a wide geographic area, including travel to sites such as Luton and Dunstable Hospital, Stevenage and Stoke Mandeville.

Qualifications

• Sound knowledge of safeguarding practice.
• Experience working with people with dementia and other long-term health conditions.
