Hertfordshire County Council’s Integrated Discharge Service is offering an exciting opportunity for a Team Manager to provide strong operational leadership within a dynamic environment, ensuring high‑quality, person‑centred practice and robust performance management to support system flow after hospital discharge.

You will oversee social workers and officers, travel to sites across Hertfordshire (including Luton and Dunstable Hospital, Stevenage and Stoke Mandeville), work on-site at least three

Qualifications

• Registered professional in social care (social work, OT or nursing)
• Full driving licence
• Flexibility to travel across Hertfordshire
• Experience managing teams in a high-demand environment
• Strong leadership with ability to deliver performance and priorities

Responsabilités

• Leading and overseeing day-to-day practice within the team, ensuring compliance with statutory duties and embedding a strong performance management culture
• Managing team performance to ensure timely discharge to assess pathways, reviews and assessments
• Inspiring, developing and managing a team of Social Workers and Community Care Officers
• Building and maintaining relationships with hospital and community partners to support integrated working
• Ensuring effective management of resources and budgets, plus quality assurance and service improvement
• Acting as lead manager for safeguarding enquiries and serious concerns
