Buckinghamshire Healthcare NHS Trust is seeking a motivated individual to join the Discharge team, supporting planning, coordination and transfer of care for patients with complex discharge needs from admission to safe discharge.

You will work under the supervision of existing discharge co-ordinators, liaising with ward staff and community services to develop timely discharge plans, advocating for patients when needed, and promoting high quality care.

Formación

• Excellent interpersonal and communication skills.
• Good time management and prioritisation abilities.
• Ability to work on own initiative as well as part of a team.

Responsabilidades

• Support delivery of high quality discharge and patient flow for complex planning needs.
• Liaise with ward staff to identify patients needing complex discharge planning and ensure referrals are made.
• Communicate with patients, relatives, carers and community services to develop and implement an optimal discharge plan.
