Job summary

Frimley Health NHS Foundation Trust is seeking an experienced Registered Nurse to join the Discharge Team at Wexham Park Hospital. The role focuses on assessment, care planning and multi‑agency working to support safe, timely discharges for patients with complex health and social care needs.

You will co‑ordinate Discharge to Assess pathways, work with wards, families and partner agencies, and promote person‑centred discharge planning to facilitate patient flow and maintain high standards of care

Qualifications

• Essential criteria: Registered nurse - NMC
• Essential criteria: ENB 998/ mentor prep or equivalent qualification
• Essential criteria: Broad clinical experience relevant to the post and experience in older people’s health
• Experience criteria: Experience of discharge coordination
• Experience criteria: Evidence of ongoing professional development
• Experience criteria: Experience of teaching
• Experience criteria: Experience of developing relationships with internal and external agencies and organisations
• Experience criteria: Experience of working across organisational boundaries within health and social care

Responsibilities

• Assisting with the co‑ordination and planning of complex discharges across all clinical specialties.
• Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.
• Working closely with patients, families, and partner agencies to ensure safe, person‑centred discharge planning.
• Supporting timely patient flow through the hospital by promoting effective discharge processes.
