VerseOne Group is seeking a bright nurse to join the Integrated Discharge team at Southend Hospital on a fixed-term basis to cover maternity leave from 22/02/2027 to 21/11/2027. The post focuses on assessing patients ready for discharge and ensuring appropriate community referrals.

The role requires excellent communication with patients, relatives, the MDT and system partners to facilitate safe discharges and maintain patient flow within the acute trust.

Responsabilités

• Assess patients medically optimised for discharge and determine post-discharge care needs.
• Refer patients to community services to support safe and timely discharge from hospital.
• Coordinate with MDT and system partners to ensure smooth patient flow and avoid delays.
• Liaise between hospital and community services to plan transfers home and back as needed.
