Job summary

• Appropriate professional social work qualification.
• Education to degree level.
• Training in outcomes focused assessment, personalisation and strengths-based practice.
• Training and experience in safeguarding.

Responsibilities

• Lead discharge planning from admission, coordinating timely discharges.
• Arrange support such as domiciliary care and residential/nursing placements.
• Carry out Care Act assessments and reviews for eligibility.
• Undertake Carer Assessments.
• Undertake Mental Capacity Assessments and contribute to Best Interests decisions.
• Undertake safeguarding enquiries and contribute to safeguarding plans.
• Contribute to risk assessment and management balancing protection and independence.
• Advise MDT colleagues on social care legislation and charging.
• Represent social work at MDT meetings and discharge forums.
• Provide advice and advocacy to service users, carers and families.
• Maintain accurate records, statutory documentation and reports; contribute to audits.
• Take part in supervision and CPD; support ASYE and students.
