Job summary

Salford City Council's Intermediate Care Social Work team seeks a capable CAO/Social Worker to support discharges from the Bevan Unit and assist with care home placements under supervision. The role involves coordinating with social care staff, residential homes, and district nursing teams to facilitate timely discharges.

You will work with adults and older people, maintaining accurate records and communicating clearly with providers and families.

Qualifications

• Ability to maintain accurate records and communicate complex information clearly, both verbally and in writing.
• Experience working with adults/older people.
• Ideally experience in health and social care.
• Previous experience collaborating with other professionals within health and social care.
• Ability to build strong relationships with care providers to foster networks.

Responsibilities

• Support discharges from the Bevan Unit under guidance from CAOs and Social Workers.
• Assist with care home finding for clients needing 24-hour care and enable prompt discharges.
