North Devon District Hospital's Care Transfer Hub seeks an experienced registered nurse to coordinate discharge planning for pathways 2 and 3, arranging onward care and supporting ward staff to clear delays. The role spans acute, community and private provider settings, with a day pattern 08:00–18:00 three days a week; full-time preferred but flexible.

You will conduct comprehensive assessments, procure onward care, and collaborate with multidisciplinary teams to discharge patients safely.

Qualifications

• Registered nurse
• Experience as a registered nurse

Responsibilities

• Complete comprehensive, holistic assessments of patients being discharged on pathways 2 and 3
• Procure onward care for patients leaving hospital
• Provide expert support to ward staff
• Resolve discharge delays
• Work closely with acute and community health and social care teams to discharge patients safely and efficiently
• Support pathway 1 discharges on occasion
