The Royal Devon University Healthcare NHS Foundation Trust is seeking an experienced Registered Nurse to join our Care Transfer Hub at North Devon District Hospital. You will assess patients being discharged on discharge pathways 2 and 3, support ward staff and help resolve delays to ensure safe, timely transfers.

You will work in an integrated MDT across acute, community and private providers. We offer flexible working options; full-time hours preferred with induction tailored to needs,

• Complete comprehensive, holistic assessments for discharged patients.
• Provide expert support to ward staff to resolve discharge delays.
• Work with acute and community Health and Social Care teams to ensure safe, timely discharge.
