Job summary

HCRG Care Group is recruiting a Team Sister for Gravesham Community Hospital to lead discharge planning within the Community Transfer of Care Hub. You will oversee day‑to‑day operations, coordinate multidisciplinary teams and work with hospitals, local authorities and community services to ensure safe, timely patient flow.

The role requires extensive post‑registration experience in discharge planning, strong leadership, and the ability to drive service improvement across health and social care

Qualifications

• Registered Nurse (NMC) or Registered Allied Health Professional (HCPC) registered.
• Degree or Diploma in a relevant healthcare profession.
• Evidence of ongoing Continuing Professional Development (CPD).
• Significant post‑registration clinical experience in discharge planning.

Responsibilities

• Provide operational and professional leadership to the Community Transfer of Care Hub team.
• Manage day‑to‑day discharge planning across pathways and oversee complex cases.
• Collaborate with acute hospitals, social care, GPs and partner organisations to remove barriers to discharge.
• Offer expert advice on funding pathways, Continuing Healthcare and safeguarding.
• Support service development, quality improvement and workforce development.
• Support recruitment, induction and professional development within the team.
