Job summary

St George's University Hospitals NHS Foundation Trust is seeking a Band 6 Admissions & Discharges Coordinator Nurse for the Cheselden Ward at St George's Hospital in London. This full-time post operates 37.5 hours weekly, with early shifts Monday to Friday and a focus on preoperative assessments and complex discharge planning.

You will lead multidisciplinary discharge planning to safely move patients, coordinate with therapy teams, social services and community partners, and help reduce length

Qualifications

• NMC registered nurse with current practice rights.
• Minimum 2 years post-registration experience.
• Experience with venepuncture and cannulation.
• Knowledge of patient pathways and discharge processes.
• Ability to assess, plan, implement and evaluate nursing care.
• Experience in vascular ward and complex discharge planning.

Responsibilities

• Admit patients preoperatively and assess discharge needs.
• Coordinate and refer to appropriate therapy team for rehabilitation.
• Arrange and chair multidisciplinary meetings for very complex discharges.
• Refer and liaise with social services and local agencies as appropriate.
• Work clinically on the ward and support ward teams as required.
