Job Overview
An exciting opportunity has arisen to cover maternity leave in the established Diabetes Nursing team! This role will be for one year fixed term and predominately be based providing care for people within the community diabetes service. The role will be based within the Community but working across an integrated nursing service using experience and specialised knowledge to provide education and advice, initiation, and adjustment to treatment plans.
The community diabetes specialist nurse will assist in the delivery of a specialist clinical diabetes service with an aim to provide high-quality diabetes care within Swindon in line with the direction of the service and Trust objectives. They will also help to deliver high-quality education to practice nurses and GPs to improve diabetes care within the primary care setting.
Detailed Job Description And Main Responsibilities
The Diabetes Specialist Nurse will be a clinical specialist, providing clinical advice, specialist skills, knowledge, and expertise to health care professionals and people with diabetes and their family/carers. The Diabetes Specialist Nurse will be committed to promoting a greater quality of life, short and long term, for the individual with diabetes and their family.
