Central London Community Healthcare NHS Trust (CLCH)

As London's largest NHS Trust dedicated to community healthcare, we offer leadership, learning and career opportunities for community healthcare professionals that few can match. We are looking to recruit a Band 7 DSN to join our dynamic team.

We are a multidisciplinary team consisting of Consultants, Dietitians, Podiatrist as well as Specialist Nurses. The team receives referrals from many primary care and community services as well as local acute hospitals.

We manage people with Type 1, Type 2 and other diabetes sub-types, initiate Freestyle Libre and Dexcom according to local guidelines, deliver DAFNE & DESMOND structured education and are a VICTOR pilot centre.

We are based in Morden (at the end of the Northern line) and work within the London Borough of Merton. Morden Underground station is just around the corner and Wimbledon Station 1.5 miles away. We are also on many bus routes, with a tram station very nearby.

Main duties of the job
  • Relevant post registration training and experience in the management of patients with Type 1 & Type 2 Diabetes.
  • Ideally be an Independent Nurse Prescriber or demonstrate a willingness to achieve this qualification.
  • Be a great team player who is willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients.
  • Have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trust's core values.
  • Be competent to offer guidance and supervision to members of the clinical team.

Person specification
Qualifications
  • Degree in Nursing or equivalent.
  • Current registration with the NMC, showing a registered General Nurse.
  • Relevant post registration training in Diabetes e.g.: Masters or diploma in diabetes management.
  • Independent Nurse Prescriber or be able to demonstrate an ability that will enable attendance of a prescribing course within 6 months of starting post.
  • Accredited trainer in DESMOND/DAFNE.
  • Supervisory skills training.
  • Post registration training in Cognitive Behavioural Therapy.
Experience
  • Extensive experience working in Diabetes either in primary or secondary care.
  • Highly experienced in managing a clinical case load.
  • Experience in the training of students.
  • Experience in organising and implementing training/teaching programmes for healthcare professionals and clients.
  • Excellent group education and presentation skills.
  • Experience of conducting audits and/or research.
  • Experience in contributing to Service Development, including policy and guideline development.
  • Experience in supervising others.
  • Experience working in a multi-disciplinary team.
  • Experience working in Community settings.
  • Experience with working with change behaviour techniques.
Skills
  • Advanced knowledge of evidence-based practice associated with diabetes.
  • Ability to interpret health and social policies and guidelines and to translate into practice.
  • Highly developed communication and report writing skills, in verbal and written format.
  • Basic IT skills – Microsoft Word, PowerPoint, Excel.
Key Attributes
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
