As a Registered Nurse, the post holder is required to adhere to NMC standards and guidelines of practice, education, and conduct.
The role involves working as an autonomous expert Diabetes practitioner delivering evidence-based care and structured education to people with diabetes, their families, and other health colleagues in a community setting. This includes participating in joint clinics and domiciliary visits for patients with diabetes, including those living in care homes with complex needs, while educating primary care staff to enhance their skills and provide excellent diabetes services.
The Diabetes Specialist Nurse will be expected to teach and support care programmes for individuals and primary care professionals, acting as a clinical resource for community nurses.
The post holder will possess specialist knowledge and skills in Diabetes Specialist Nursing, serve as a resource for team members, support clinics across the locality, and participate in clinical supervision.
The role involves working with colleagues in primary and secondary care to develop an integrated, research-based care delivery system.
