Job description

This is an exciting time to join the Community Nursing service in West Hertfordshire. We are developing new community pathways which will support the delivery of more care in people's homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy. Key duties within the role include:

  • To set and maintain standards of care and deploy, co‑ordinate and supervise the nursing team and learners as required.
  • To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
  • To be responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-6pm.
  • To be responsible for the team's maintenance of patient and other records.
  • To participate in the long‑term management of patients within the locality.
  • To have continued responsibility for a defined caseload.
  • To be responsible for the development and maintenance of competencies within the team.

We are a dynamic and forward‑thinking team looking for an experienced and motivated Band 6 Registered Nurse to join our established St Albans & Harpenden Core Neighbourhood Nursing Team as a Deputy Team Lead. This role offers an excellent opportunity to co‑lead one of our neighbourhood teams, working closely with and supported by experienced Team Leaders. The position is varied and rewarding, with responsibility for managing a patient caseload. You will assess, plan, implement and evaluate evidence‑based care for patients in their own homes, helping to support independence and improve outcomes. We are particularly interested in candidates with previous community nursing experience. We are looking for someone who is committed to collaborative working, approachable, and an effective communicator. You will be confident working both independently and as part of a team, and open to new ideas and ways of working. As this is a community‑based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010). Applicants are expected to present clear and relevant evidence of the competencies and responsibilities detailed in the attached Job Description and Person Specification, together with a demonstrated commitment to the Trust's values of Accountability, Inclusion, Compassion, and Empowerment.

We are proud to be one of the largest community healthcare providers in the country, with more than 4,500 colleagues caring for over four million people across London and Hertfordshire. Every day, our teams bring their skill, compassion, and determination to the people who depend on us. What inspires us is at the heart of who we are: when we work together, we can help people move forward in ways that truly matter. Our teams support children as they take their first steps in life, and they stand beside adults as they rebuild strength, confidence, and independence. From newborn health visiting to community nursing, stroke rehabilitation, and palliative care, we are there for people through some of life's most important moments. Joining Central London Community Healthcare means becoming part of a community that lifts each other up. It means working in an organisation that values compassion, welcomes new ideas, and believes in the potential of every colleague. Your development matters here. Your wellbeing matters. Your voice helps shape the future of the care we provide.

We offer a competitive employment package because the work you do matters. At Central London Community Healthcare, you will join an inclusive organisation that invests in its people, supports development, and helps you thrive while delivering high‑quality care.
