Job summary

Central London Community Healthcare NHS Trust in Watford is seeking an experienced Band 6 Registered Nurse to join the Hertfordshire Adult Community Services as a Deputy Team Lead.

You will co-lead a neighbourhood community nursing team, manage a patient caseload at home, and drive evidence‑based care. A full UK driving licence and car for business use are required where eligible.

Qualifications

• Registered Nurse with current UK NMC registration.
• Practice Supervisor/Assessor or equivalent.
• Specialist Nursing Practice (District Nursing) Degree or equivalent training.
• Nurse Prescribing or willingness to undertake training.
• Evidence of ongoing personal development and NMC code knowledge.
• Independent Prescriber experience or training planned.

Responsibilities

• Lead and co-ordinate the nursing team across Hertfordshire localities.
• Assess, plan, implement and evaluate individualised home-based nursing care.
• Maintain standards of care and supervise learners and staff.
• Manage a defined caseload and ensure seven days a week service delivery.

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

We are a dynamic and forward-thinking team looking for an experienced and motivated Band 6 Registered Nurse to join our established Watford and Three Rivers Community Nursing Team as a Deputy Team Lead.

This role offers an excellent opportunity to co-lead one of our neighbourhood teams, working closely with and supported by experienced Team Leaders.

The position is varied and rewarding, with responsibility for managing a patient caseload. You will assess, plan, implement and evaluate evidence-based care for patients in their own homes, helping to support independence and improve outcomes.

We are particularly interested in candidates with previous community nursing experience.

We are looking for someone who is committed to collaborative working, approachable, and an effective communicator. You will be confident working both independently and as part of a team, and open to new ideas and ways of working.

As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

This is an exciting time to join the Community Nursing service in West Hertfordshire. We are developing new community pathways which will support the delivery of more care in people’s homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy.

Key duties within the role include:

• To set and maintain standards of care and deploy, co‑ordinate and supervise the nursing team and learners as required.
• To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
• To be responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-6pm.
• To be responsible for the team's maintenance of patient and other records.
• To participate in the long-term management of patients within the locality.
• To have continued responsibility for a defined caseload.
• To be responsible for the development and maintenance of competencies within the team.
