Job summary

Cheshire East Council is seeking an experienced Care Worker to join the Care4CEDementia Reablement Team for a six-month temporary role at 37 hours per week. Based in Macclesfield Town Hall, you will visit referrals at their homes and use a reablement approach to support adults with dementia and their families.

You will have a background in social care or health care, with dementia experience, and the ability to develop strong relationships with service users and carers.

Qualifications

• Experience of working in a Social Care or Health Care setting ideally with experience of supporting referrals with dementia.
• Experience of supporting service users and carers and developing good working relationships.

Responsibilities

• Support referrals with dementia and provide practical and emotional assistance to service users.
• Support carers and families and maintain good working relationships with them.
• Manage caseload and make referrals for OT, Assistive Technology and carer assessments.
