Job summary

Cheshire East Council is recruiting for a permanent 1 FTE Social Care Assessor to join the Care4CE Mental Health Reablement team in Crewe. You will perform needs-based assessments and plan short-term practical support to adults experiencing mental health or related issues, aiming to maximise independence.

The role requires safeguarding knowledge, good communication, organisation, and record-keeping. The team operates mainly 9am–5pm, with some early evenings; based at Delamere House, Crewe,

An exciting opportunity has become available to work within the Care4CE Mental Health Reablement team in Crewe. The position is for 1 FTE (37hrs) permanent Social Care Assessor.

The team primarily work Monday to Friday 9am - 5pm, although some early evenings may be required. The service is based at Delamere House in Crewe but covers the south of Cheshire East.

About the role

The Mental Health Reablement team provides short term (up to 6 weeks) practical support to adults who are experiencing mental health issues and/or drug and alcohol related issues. This may include support with debt management/budgeting, housing issues, education, employment, social inclusion, leisure activities, etc. The support focuses on coping techniques, increasing self-esteem and confidence, promoting social inclusion and goal setting to support people to maximise their independence. The successful candidate will be expected to complete needs-based assessments for customers referred to the team, to identify and agree the support that the team will provide taking into account any risks, safeguarding concerns, etc.

The successful candidate will:

• Have experience of working in a health or social care setting and have an understanding of mental health issues.
• Be able to assess risk and demonstrate a clear understanding of safeguarding procedures.
• Demonstrate a knowledge of local service provision.
• Have excellent communication, organisation and record-keeping skills.
• Be enthusiastic, flexible, reliable.
