Job summary

Cheshire East Council is seeking an experienced Care Worker to join the Care4CEDementia Reablement Team for a six-month temporary role at 37 hours per week. Based in Macclesfield Town Hall, you will visit referrals at their homes and use a reablement approach to support adults with dementia and their families.

You will have a background in social care or health care, with dementia experience, and the ability to develop strong relationships with service users and carers.

Job description

Anopportunity has become available to work within the Care4CEDementia Reablement Team. The team offer support to adults with mental health issues using the reablement model. This is a temporary position for six months for Grade 6 for 37 hours a week. The hours are primarily 9am – 5pm, although some early evenings may be required and flexibility is needed. The service is office base is Macclesfield Town Hall, but the role requires being about to visit our referrals at their homes and being able to transport to community resources, a mileage rate is paid for the use of your own vehicle.

About the Role

The referrals to the team mainly are from the Memory clinic after they receive the initial diagnosis, our role is to support them practically and emotionally. The worker will support carer and families too. The workers have a caseload to offer support and make referrals for example for OT, Assistive Technology and carer assessments.

About you
The successful candidate will have:
  • Experience of working in a Social Care or Health Care setting ideally with experience of supporting referrals with dementia.
  • Experience of supporting service users and carers and developing good working relationships.
