Job summary

Hatched Recruitment Group is recruiting Reablement Support Workers for an outstanding reablement service in the local community. You will help adults regain independence and confidence while living at home and in their communities.

You will work as part of a multidisciplinary team, encouraging people to develop skills and independence after illness or hospital discharge, following individual care plans and rehabilitation plans. This temporary position runs for 12 weeks with potential extension.

Qualifications

  • Experience in care or support environment.
  • Level 2 Health & Social Care or willingness to work towards it.
  • Experience supporting personal care and daily living activities.
  • Knowledge of medication safety and administration.

Responsibilities

  • Provide personal care and support with daily living activities.
  • Assist mobility and safe movement.
  • Support meals, nutrition, and hydration.
  • Support safe medication administration and management.
  • Follow care plans, risk assessments and rehabilitation plans.
  • Assist with therapy/exercise programmes after training.
  • Use mobility and care equipment safely.
  • Care for catheter and stoma needs with training.

Job description

Reablement Support Worker

We are recruiting compassionate and dedicated Reablement Support Workers to join an Outstanding-rated reablement service working on behalf of a local authority.

This is a fantastic opportunity to make a genuine difference by supporting adults to regain their independence, rebuild confidence and continue living safely within their own homes and communities.

The Role

As a Reablement Support Worker, you will work as part of a multi-disciplinary team supporting adults following illness, injury, hospital discharge or changes in their circumstances.

Rather than simply completing tasks for individuals, you will encourage and empower them to develop their skills, confidence and independence.

Your responsibilities will include:
  • Providing personal care and support with daily living activities
  • Supporting individuals with mobility and safe movement
  • Assisting with meal preparation and nutrition
  • Supporting the safe administration and management of medication
  • Following individual care plans, risk assessments and rehabilitation plans
  • Supporting individuals with therapy and exercise programmes following appropriate training
  • Using mobility equipment and other care-related equipment safely
  • Supporting individuals with catheter and stoma care following appropriate training
  • Promoting independence, dignity, choice and wellbeing
  • Encouraging community participation and social inclusion
  • Monitoring changes in individuals' needs and reporting concerns appropriately
  • Maintaining accurate care records and documentation
  • Working independently in the community and using your own initiative
  • Working closely with families, carers and other health and social care professionals
What We're Looking For

We are looking for caring and reliable individuals with a genuine passion for helping people achieve greater independence.

Ideally, you will have:
  • Experience working in a care or support environment
  • A Level 2 qualification in Health and Social Care, or equivalent, or a willingness to work towards this
  • Experience supporting individuals with personal care and daily living activities
  • Knowledge of supporting people with medication safely and accurately
  • An understanding of person-centred care and promoting independence
  • Experience of working with people with disabilities, reduced mobility or life-changing conditions
  • Knowledge of manual handling and relevant care equipment
  • Experience of lone working and using your own initiative
  • Good verbal and written communication skills
  • Basic IT skills
  • An understanding of safeguarding, health and safety, equality and diversity
  • The ability to adapt to changing circumstances and follow instructions effectively
Working Hours

This role is for 25 hours per week, working on a rota basis.

You will be required to work flexible shifts between 8:00am and 10:00pm, seven days a week, including weekends. Shifts will predominantly include early mornings and evenings.

Additional Information

This is initially a 12-week temporary assignment, with the potential for extension while permanent appointments are being made.

The role involves supporting people across the local community, helping them remain safe, independent and connected to the people and places around them.

An Enhanced DBS check, including an Adults' Barred List check, will be required for this position
