Job summary

Derbyshire Healthcare NHS Foundation Trust is recruiting for a Band 3 Occupational Therapy Assistant to join the Dementia Rapid Response Team covering South Derbyshire and Derby City. The role provides home-based crisis intervention and supports early discharge planning, with a focus on independence and social inclusion in home and community settings.

Qualifications

• Essential: NVQ Level 3 or equivalent
• Essential: Experience of working with Older People
• Essential: Experience of working within Mental Health Services
• Desirable: Experience of working with people who have a Dementia
• Essential: Good communication skills
• Essential: Knowledge of functional illness
• Essential: Hold a full UK driving licence

Responsibilities

• Provide timely crisis response and de-escalation in acute mental health crises.
• Contribute to person-centred care plans and monitor changes in occupational functioning.
• Identify safeguarding concerns and follow procedures to protect vulnerable adults.
• Educate and support families and carers to aid recovery.
• Work with colleagues to deliver an effective and safe service.
• Support the Occupational Therapist with assessment and treatment.
• Collaborate with the ward team to maintain patient and family safety.
