Derbyshire Healthcare NHS Foundation Trust is recruiting a Lead Nurse for the Dementia Rapid Response Team in Derby. The role delivers person-centred, non-pharmacological care to older adults with dementia in the community.

You will work with an MDT to prevent hospital admissions, travel across the catchment, and cover a 7-day service pattern with a 12-week roster, supported by supervision and training.

The post is permanent, full-time, with salary £39,959–£48,117 per year pro rata.

Qualifikationen

• Recent and relevant, post-qualification nursing experience as RMN.
• Community experience.
• Experience working with older adults.
• Experience in working with people with Dementia.

Aufgaben

• Lead a MDT in delivering a flexible dementia care service in the community.
• Make complex decisions, lead a shift, cover duty, and formulate assessments.
• Develop person-centred care plans to prevent hospital admissions.
• Coordinate short-term intensive interventions with a multi-disciplinary team.
• Ensure documentation using EPR and maintain high-quality records.
• Provide personal development through supervision and training opportunities.

Kenntnisse

Knowledge of MHA/Capacity act/Legal  

Able to meet travel requirements

Ausbildung

Mentorship/SSSA trained or willing to complete training

Supervision trained
