Job summary

Derbyshire Healthcare NHS Foundation Trust is seeking a Lead Nurse for the Dementia Rapid Response Team in Derby. The role is Band 6, full-time (37.5 hours) with a focus on dementia care in the community and hospital discharge support.

You will work within a MDT, lead shifts, assess patients, and apply MCA/MHA knowledge. The service operates across Derby City and South Derbyshire, promoting non-pharmacological care and person-centred plans.

Qualifications

• RMN with post-qualification nursing experience.
• Mentorship/SSSA trained or willing to complete training.
• Knowledge of MHA/Capacity Act and legal frameworks.
• Willing to travel across the catchment area.

Responsibilities

• Be clinically responsible for a flexible, responsive local service.
• Travel independently across the catchment area as required.
• Work a shift pattern across 7 days per week.
• Undertake the Duty role; triaging referrals and delivering handover.
• Work as part of an MDT with various professionals.
• Complete initial assessments of patients.
• Apply MHA and MCA where required.
• Formulate person-centred care plans with non-pharmacological interventions.
• Provide in-reach to inpatient wards and attend 117 meetings.
• Visit patients at home and in care settings.
• Utilise the EPR for documentation.
